Home- is to enable an individual to remain in the familiar surrounding of his/her own home as long as is possible and appropriate, focusing on:
• Maximize patient independence
• Deliver services at the convenience of the patient and family and caregiver
• Arrange respite services for caregivers
• Bridge gaps in the patient’s caregiving network
• Adapt the home environment to meet the patient’s physical needs

Facility Based Care- Because a person’s home may not be his/her personal residence, a hospice program may, as an alternative, offer home care services in a variety of facility settings appropriate to the patient’s care needs. The hospice is responsible for ensuring that an appropriate standard of care is provided in the facility regardless of whether or not the hospice is responsible for the direct provision of those services, e.g. room and board services in a nursing facility.
