Hospice encourages the patient and family to participate in developing the interdisciplinary team plan of care and in the provision of hospice services. The plan of care includes specific goals for involving the patient and family, including family that may live outside the geographic area.

Each patient and family is provided with a statement of their rights and responsibilities. A written consent is required of the patient and family for clarification that they are seeking care. Documentation supports the ongoing instructions and support provided to the caregivers.
