Access to Hospice Care

Based on an annual, documented needs assessment, the hospice engages in outreach programs appropriate to the hospice needs of people in the communities it serves.

Hospices must establish admission and discharge criteria which clearly state requirements and any eligibility limitations. Any eligibility limitations are evaluated on an annual basis with the goal to increase access to care in the community.

Hospice programs will provide to the community and referral sources information about the services it offers, who qualifies for services, and how services may be obtained. Hospices will provide a variety of planned community education activities. Access to care is facilitated by providing services, staff, and management sensitive to the culturally diverse needs of the communities it serves. Hospice will make every opportunity to entitle the community to the programs benefits.
